Required I understand that I must accurately inform Cruise Holidays and Jennifer Gonzalez of my travel companions’ and my full legal names, birthday, and passport information prior to booking my vacation. I understand that if I fail to provide Cruise Holidays and Jennifer Gonzalez with accurate information about my legal name, birthday, or passport information that I may incur additional expenses for updating my name, birthday, or passport information on my travel reservations
Required I understand that most cruises require me to have a passport which is valid for up to 6 months after the end of my cruise. I assume all responsibility for obtaining my passport by the time of my cruise departure. If I do not obtain my passport by the time I depart for my cruise I will not hold Jennifer Gonzalez or Cruise Holidays responsible for any additional expenses, cancellation fees, or change fee which I may incur due to not having a valid passport upon the departure of my cruise. I may not take legal action against Jennifer Gonzalez or Cruise Holidays if I do not have a valid passport by the time I depart for my cruise. I understand that if the cruise which I have purchased does not require me to have a passport, it’s still HIGHLY RECOMMENDED to have a passport which is valid for 6 months beyond the end of my cruise. I assume all risks associated with traveling without a passport and will not hold Jennifer Gonzalez or Cruise Holidays responsible for any complications, issues, or difficulty I may incur while attempting to re-enter the United States without a passport. I accept all responsibility for any additional expenses I may incur due to traveling on a cruise without a passport.
Required I have read the cancellation fees noted on my travel agency confirmation. I understand that I will be responsible for those cancellation fees under any and all circumstances no matter what. I understand that I have the option to purchase travel insurance and that Jennifer Gonzalez recommends that I purchase travel insurance to protect my vacation and the money which I spent on the vacation that I I booked through her travel agency.
If I do not purchase travel insurance:
Required I understand that any and all money that I paid for my travel/vacation package or travel related expenses can be forfeited to Cruise Holidays or any vendor used for my travel planning if I need to cancel or modify my travel plans. I understand that even if I have an extreme situation (for example a death in my family) that the vendor with which I reserved my travel/vacation package may not refund my money for the travel/vacation package.
Required I understand that Cruise Holidays and Jennifer Gonzalez will make all available efforts to help me receive a refund from a vendor if needing to cancel or modify my trip/vacation package. However, I understand that the efforts of Cruise Holidays and Jennifer Gonzalez may not allow me to be refunded the money which I spent on a trip/vacation package if I need to modify or cancel it.
Required I understand that my travel/vacation package or other related travel expenses may have to be modified by the vendor due to circumstances outside of the control of the vendor or Cruise Holidays (an example of this would be natural disasters or terrorist activities). I understand that this may result in me forfeiting money that I spent on this travel/vacation package or travel related expenses.
Required I understand that if I choose not to purchase travel insurance for my travel/vacation package that I’m responsible for any unforeseen expenses related to my travel/vacation package; an example of unforeseen expenses would be weather delays or medical/emergency expenses.
Required I understand that I have the option to purchase travel insurance for my travel/vacation package through Cruise Holidays and Jennifer Gonzalez. I also understand that Cruise Holidays HIGHLY RECOMMENDS that I purchase travel insurance even though it isn’t a required purchase when purchasing travel/vacation package through Cruise Holidays and Jennifer Gonzalez.
If I do purchase travel insurance:
Required I understand that travel insurance will probably reimburse me for most but not all cancellation fees or modification fees prior to travel. I also understand that travel insurance will probably reimburse me for most but not all unexpected expenses acquired during travel for unforeseen circumstances; some examples of these are: weather issues, travel delays, medical issues, and emergencies.
Required I understand that when I purchase travel insurance and the type of insurance that I purchase will affect what expenses are covered by my travel insurance.
Required I understand that if I have additional travel expenses added to my travel/vacation package after purchasing the travel insurance policy that I must notify my travel agent of these additional expense and add the expenses to my insurance policy for them to be covered under the policy. I also understand that adding travel expenses onto my travel insurance policy will result in an increased cost to my insurance policy.
Required By checking this box, I agree to the above terms for any and all future travel/vacation packages I will purchase through Jennifer Gonzalez and Cruise Holidays after the below listed date and in addition to the above listed travel/vacation package.
Required By checking this box I authorize my Travel Agent, Jennifer Gonzalez, to charge my credit card for my current vacation and any future vacations with which I will book with her and/or her travel agency. I'm giving her permission to charge my credit card to complete payments for the cost of my vacation package from a primary vendor, subsequent/additional vendors, and/or by my travel agency.
